 We do not currently have a secure website that would enable us to collect this information directly over the internet – please print the form at the link below and mail it to us at the address listed on the form.  This not only helps fund the newsletters and programs we provide, it also gives us the opportunity to keep our mailing list up to date.  Thank you for supporting the Down Syndrome Community!
